How to Enroll Your Child At Palominas School District
● Please fill out a complete enrollment packet
● Please check with your physician that your child is up to date on their shots, and provide a copy of their updated Immunization Record
● Please provide a copy of their Birth Certificate
● Please provide proof of your family’s residence in the state of Arizona (eg. Driver’s License, Rental Agreement, Utility Bill etc.)
● If you are the child’s Legal Guardian, please provide copies of relevant court documents appointing you as Guardian
● Please provide copies of any other court documents you wish the school to have access to (relating to custody, etc.)
